Can you spare 5 minutes for our survey?
If you are an annual member of Countryside Mobility we are inviting you to take part in a short survey about your experience of the Countryside Mobility service.
Your feedback will help shape the future development of Countryside Mobility and ensure it continues to meet the needs of its members. The survey takes around five minutes to complete and includes 13 questions. All responses will be treated confidentially.
As a thank you, you can enter our prize draw by providing your membership number at the end of the survey. Four prizes are available. The first two names drawn will each receive a £15 Countryside Mobility Annual Membership voucher, and the third and fourth names drawn will each receive a £15 Marks & Spencer gift voucher*

- The prize draw is open to current Countryside Mobility members aged 18 and over. Membership fees apply.

- The winner(s) will be selected at random from all eligible entries received by the closing date.
- There are no cash alternatives or alternative prizes available.
- Winners will be notified by email within one month of the survey closing date.
- If a winner does not respond within 14 days of notification, Countryside Mobility reserves the right to select an alternative winner.
- The organiser’s decision is final and no correspondence will be entered into.
- Personal data supplied will only be used for the administration of the prize draw and handled in accordance with Countryside Mobility’s Privacy Policy.
- The organiser reserves the right to amend, suspend, or withdraw the prize draw where necessary due to circumstances beyond its reasonable control.
